Health Professionals
An electronic referral form is provided for your convenience. If you would like a paper referral pad please get in touch. Please feel free to contact Lorraine Favaretto by phone or email if you would like to discuss cases in advance or give further information. Medicare Team Care Arrangements are accepted but the rebate will not cover the full cost of an appointment. Unfortunately DVA referrals are not accepted at this time.
ELECTRONIC REFERRAL
If your patient consents to their details being sent electronically, complete the form via the link below, click submit and the form will send automatically.
PAPER REFERRAL
Access the form via the link below, the completed referral form can be printed and given to your patient to bring to their appointment or emailed in advance. Please ask if you would like a referral pad to be sent to you.

Without a Referral / Self Referrals
A referral from a health professional is not required. If you have any symptoms relating to your head, neck or jaw, you can book directly online or please get in touch to discuss your needs Lorraine, the clinic physiotherapist based in Margate, QLD. If booking online please include a note in the comments section stating what type of problem you would like help with.
