Health Professionals
An electronic referral form is provided for your convenience. Please feel free to contact Lorraine Favaretto by phone or email if you would like to discuss cases in advance or give further information. Medicare Team Care Arrangements are accepted but the rebate will not cover the full cost of an appointment.
ELECTRONIC REFERRAL
If your patient consents to their details being sent electronically, complete the form via the link below, click submit and the form will send automatically.
PAPER REFERRAL
Access the form via the link below, the completed referral form can be printed and given to your patient to bring to their appointment or emailed in advance.

Self Referrals
A referral from a health professional is not required. If you have any symptoms relating to your head, neck or jaw, please contact us to discuss your needs with our physiotherapist based in Redcliffe. If booking online please include a note in the comments section to let us know what type of problem you would like help with.
